It started before your periods did

The brain fog, the broken nights and the mood that is not yours arrive years before anything stops. That is why you were tested, told you were normal, and sent home with all of it still happening.

You are somewhere between 42 and 52. Your periods are still coming, more or less. And yet the words go missing mid sentence, you are awake at three most nights, your temper is not yours, and your body has changed shape on the same food.

This is the part of menopause that happens while you are still having periods. It has a name, it is measurable, and it is the single most under recognized phase in women's medicine.

Almost every woman who writes to this site says a version of the same sentence. I know something is wrong and nobody will take it seriously.

She has usually been to a doctor. She has usually been tested. She has usually been told the results are fine. And she leaves with every symptom she walked in with, plus the new suspicion that she is imagining it.

What the years before actually look like

The menopausal transition is not the day your periods stop. It is the several years before that day, and it is where most of the disruption lives. Women describe it in this order, and rarely to the same person:

Presented one at a time to different doctors, each of these gets its own explanation: stress, age, work, the children, a bad year. Presented together, to someone who works in hormones, they are one story.

Why the blood test said you were fine

Here is the fact that resolves most of the confusion. In the years before your last period, estrogen does not decline gently. It swings. It can run higher than it did in your thirties one month and fall away the next.

So a single blood sample tells you what was happening in your bloodstream on one particular morning. It does not tell you what has been happening for two years, and it cannot tell you what is coming. A normal result on a bad day is common, and it is not reassurance.

This is why the diagnosis in this phase is made mostly from the pattern of what is happening to you, not from a number on a form. Getting that right is a specialist skill, and it is the difference between being believed and being dismissed.

What is at stake while you wait

Waiting is a decision, and it is not a neutral one.

Bone loss runs fastest from about a year before your final period to two years after it, which is precisely the window in which almost nobody is scanned. Body composition shifts during the same years, fat moving inward and muscle falling away, and with it the risk of type 2 diabetes rises. The intimacy symptoms are different from the hot flashes in one important respect: they do not fade with time, they advance.

And underneath the medicine there is the plain fact that these are years of your life. Sleeping through the night, thinking clearly, wanting things again, recognizing your own temper. Those are not luxuries and they are not vanity.

You do not need to prove you are ill to be entitled to a proper assessment. You need somebody who knows what to measure, in what order, and how to read it against what is actually happening to you.
